Hours

We are open to serve you, answer questions or schedule an appointment during the following hours:
7:15 am to 5:30 pm, Monday-Thursday

Appointments

Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to change your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged a missed appointment fee.

Insurance

We will fully attempt to help you receive maximum insurance benefits: however, you are personally responsible for your account. Because insurance policies vary, we can only estimate your coverage in good faith, but cannot guarantee coverage due to the complexities of insurance contracts. We ask that you pay your estimated patient portion at the time of service. As a service to our patients, we will bill insurance companies for treatment rendered.
